Your First Visit

YOU WILL BE ASKED TO COMPLETE A NEW PATIENT HEALTH HISTORY FORM ON YOUR FIRST VISIT. 
​

  • Please arrive 15 minutes early to complete your initial visit paperwork. Alternatively, the initial visit paperwork can be found under the forms section, or we can email you the paperwork to complete prior to your first appointment.

​When you call or email to schedule our appointment, 
Please ensure that you have your insurance information with  you.
That would be all the current primary and secondary insurance information, this includes the policy number, group number and certificate number.
Please make sure that you know who is the primary insured for each account  if there is more than one), and who is covered under each plan.

  • Please have the name, address and contact information for you last dentist/dental specialist.  Please let us know if you have been to see them in the current calendar year.
 
  • If there are X-rays on file for you at your past dental clinic, we will work to obtain the records, and /or take new ones as required.
  • You will either need to sign a release at our clinic, or at the previous clinic, in order for us to obtain these records for you. You can find a copy of this release under forms. 



​​At your appointment, we will complete a full oral health exam in order to help determine your dental needs and review your past medical and dental history.

Please allow 45 minute to 1 hour for your first appointment,

This will help us to create a personalized and comprehensive dental plan.

We want to get to know you to help provide you with the best possible care. It helps us to  know your preferences and how best to answer your questions.

We respectfully request that you give 2 working days notice if you must cancel an appointment. This is to provide an opportunity for other patients and clients to book during that time slot.  We reserve the right to charge for missed or cancelled appointments without sufficient notice.

If you have extended health benefits, we will coordinate the direct billing to your insurance company,
​ You are responsible for any co-payment not covered by your insurance company.
